I think this was a good poll; interesting. It asks about something almost all people have operating at the moment they're considering taking the poll, which leads to interest about how many tabs others have open as well.
You ask whether "I am more likely to click an advertisement if it fits well into the layout of the page, as opposed to one that is obtrusive or is obviously an advertisement.". I voted "False" on this one, and I think most people would too if they had thought about the question in depth.
I think that anyone who has used the internet for any amount of time acquires the ability to selectively ignore advertisements. For example, I did not notice the Google Adwords advertisement on the left side of this website until I read the statement in your poll and thought about it. It is so integrated into the layout that it just seems to fit, but as soon as I see that it shares the common layout of a Google ad and contains no information relating to polls, I subconsciously ignore it.
The advertisements I do notice however are those which don't fit. Whether the advertisement blinks, animates, makes sounds, or seems like a game, it is always noticeable. However, whenever I do see such an ad, I never consider visiting the website, for I feel any company which resorts to these annoying gimmicks is untrustworthy and is only after my money.
In the 10 years I've been using the internet, I've only once (purposefully) clicked on an ad because the product it offered interested me. It was an advertisement for Wicked Lasers™ high intensity laser pointers, and the ad was placed on the end of a website which about electronic gadgets. It was a Google Adwords ad -- very plain, white with text. I never did buy the product because I saw no redeeming use for a high intensity laser pointer in my life.
You forgot Czech Repulic. (Which I have been to, BTW)
