Diaper/Baby Punishment Follow-Up

This is is a follow-up to my earlier poll on this subject. This poll addresses questions brought up by the results of and comments on the first poll as well as new questions that I have. I strongly encourage people to post or e-mail me, especially if asked to by a question. I am mot going to criticize or make fun of anyone. Serious responses only please (i.e. please no obvious fantasies).
At what age was diaper/baby punishment first used?
