Mister Poll: Obtaining medical care in the U.S.

I don't live in the U.S., but I was just wondering what people who do live there really think about the way medical care is treated there.

When it comes to being able to afford medical care, the following are affordable to practically any U.S. citizen with a stable job (pick one or several):

Check-ups

Emergency room visits

Delivering a child

Surgery

Cancer treatment

Pharmaceuticals

None. They're all expensive.

Do insurance companies make things more or less affordable for the average person to have medical care?

When I hear of a rich person getting elective plastic surgery, while some poorer people can't (or can barely) afford needed surgery or medication, I think:

That's America, you've got to earn your way, that's how things should be.

That's terrible. We're rich and advanced enough that certain neccessary medical care should be given to all citizens.

A lot has been said about insurance companies denying coverage or even needed medical care to people covered in order to save money, that President Nixon was happy to see a system in which companies got rich by denying medical care. I think that is:
